Community Health Assessment aNd Group Evaluation (CHANGE): Building a Foundation of Knowledge to Prioritize Community Health Needs
The CHANGE tool helps public health workers understand the policy, systems, and environmental change strategies in place in their community, and then develop a community action plan to ensure those strategies support healthy lifestyles.
CHANGE was developed to assess the risk factors that contribute to chronic diseases in five community settings, such as health care, schools and worksites, and create an action plan. The following resources can be downloaded from the website:
